Revere, MA for 32 hours/week.
Brighton, MA for 20 hours/week

Position includes:
*Â Â Â Completing clinical nutrition assessments
*Â Â Â Documenting using the Nutrition Care Process in the EMR system
*Â Â Â Collaborating with the interdisciplinary care plan team
*Â Â Â Completing the MDS – Section K
*Â Â Â Calculating TF
*Â Â Â Making recommendations for wounds and significant wt changes
Requirements:
*Â Â Credentialed by the CDR
*Â Â Dietetic Licensure
*Â Â Positive attitude
*Â Â Client Centered
*Â Â Good communication skills
