Highlights of the Role:
- Schedule: 16 hours/week (each location)
- Duration: Open-ended
- Facility Type: Long-term care facilities
- Start Date: June 2025 (possibly July)
- Drive Time: Offered for qualifying candidates
- Experience: LTC and MDS experience preferred (not required)
- EMR: Point Click Care (all facilities)
Your Assignment Will Include:
- Completing nutritional assessments and maintaining documentation
- Documenting with the Nutrition Care Process in the electronic medical record system
- Collaborating with the interdisciplinary care plan team and food service team
- Calculations and recommendations for enteral feedings
- Making recommendations for wounds and significant weight changes
- Completing MDS – Section K
- Maintaining confidentiality and patient information in accordance with HIPAA
Requirements:
- Credentialed as a Registered Dietitian Nutritionist (RDN) through the CDR
- Licensed in Massachusetts (must be active prior to start date)
- Must pass a pre-employment screening process, including background and health checks
