Promote hope, empowerment and recovery in all aspects of care. Empower Persons to exercise autonomy in all aspects of their life.
- Assist Persons with identifying choices and developing skills in self advocacy. Model professional behavior for staff and promote use of recovery based language.
- Provide overall clinical supervision and oversight of service delivery of up to 28 Persons serviced.
- Provide clinical guidance and support in developing interventions and supervision of team members. Provide direct supervision to all staff both in the office and in the field.
- Teach and model clinical interventions, strength based strategies and ensure services are culturally competent for Persons served.
- Ensure that services are culturally competent, strengths based, recovery based, focused on rehabilitation and are inclusive of Person preferences. Ensure that the Person is at the center of all care and that all interventions are developed and provided in a manner consistent with Person preferences.
- Complete all intakes of new Persons, complete required assessments, obtain authorizations, complete crisis plans and coordinate aspects of Persons admission according to agency and DMH policies.
- Complete assessments for all individuals on caseload during the first 45 days of services providing strengths based in accordance with Eliot policy, rehab option and DMH standards.
- In collaboration with the Person, natural supports, and the Team, develop Treatment Plans (TPs) that include the Person’s goals and prioritized needs from the assessment.
- Guide the clinical direction of the TP and interventions developed.
- Ensure that the TP is person centered and fully inclusive of the Person’s preferences. Ensure that treatment goals are realistic, attainable and embrace recovery and rehabilitation.
- Ensure that each staff member is actively engaging Person’s TPs, interventions and life goals. Oversee staff members overall adherence to clinical interventions, cultural preferences and guide staff in specific evidenced based practices.
- Monitor progress towards goals through supervision of staff members and regular contact with Persons.
- Provide side by side interventions to Persons related to their TP and support recovery and rehabilitation.
- Provide clinical direction and clinical interventions to support treatment plan process.
- Ensure that Person’s strengths and motivations are central to goals and interventions, ensure family and natural support involvement in the assessment and treatment planning process consistent with persons preferences.
- Assist Persons in developing and practicing self-management skills, coping skills and clinically based skill development.
- Where appropriate engage Persons in CBT, DBT, integrated dual diagnosis treatment, self-management plans, and other clinical interventions. Provide groups where appropriate and indicated for Persons.
- Oversee staff members’ utilization of community resources and natural supports in working with Persons. Ensure the inclusion of these supports and linkages in TP and daily interventions.
- Monitor/Supervise issues related to acuity and work with Persons and staff members to ensure that acuity is responded to in a timely manner. This should include increased interventions, or changing interventions and providing direct clinical support to Persons and staff.
- Continuously assess training needs of staff and provide on-going training to staff members related to observations of directed by Team Leader or Assistant Team Leader.
- Promote concepts of health and wellness. To assist Persons with accessing information and services related to medical care, nutrition, exercise and smoking cessation as appropriate. To assist Persons with accessing community resources that supports these healthy areas.
- Act as the clinical back up on call supervisor to Intensive Case Manager and Residential managers. Assist with issues pertaining to Person care, medications, psychiatric/medical crises and acuity. Provide direct clinical support to Persons where needed to avoid use of emergency rooms and emergency services unnecessarily.
- Ensure the safety of Persons; implementing crisis intervention as necessary. Includes but not limited to: use of deescalating modes of communication/ interaction to ensure Person’s safety, stability, mental status, and physical health; utilize supervisor and team members for consultation; communicating all pertinent information in a clear and concise manner with particular attention to clinical observations.
- Participate in the Team Huddle, Mini Team and Team Meetings in accordance with Eliot policies and ACCS standards. This shall include completing progress notes prior to the Team Huddle for every Person contact and collateral contact in the format indicated by policy.
- Redeploy staff members in order to cover missed appointments or respond to acuity, and communicate with Team Leader or Assistant Team Leader around any crises.
- Provide specific interventions indicated on Person’s treatment plan as part of Mini Team and in support of Team activities.
- Maintain and adhere to Agency policy/procedures regarding Person confidentiality. Ensuring that: Person records are maintained in a secure manner and access is limited; clinical conversations occur in an area where the information can not be overheard by others; privileged information regarding a consumer is not given verbally or otherwise without the consumer’s written consent. Adhere to all HIPAA regulations.
- Ensure that Persons are treated with dignity and respect in accordance with ECHS’ Human Rights policy. Ensure that that the foundations of services compasses empowerment, choices and recovery. Ensure the Persons are free from neglect and abuse. Report all concerns/ suspicions of abuse/neglect to supervisor and the Disabled Persons Protection Commission, Executive Office of Elder Affairs, or Department of Children and Families.
- Act as a liaison to DMH, other providers, community resources, family members and others that may be involved in a Person’s care.
- Responsible for promoting and upholding the principles of Corporate Compliance and the ECHS Corporate Compliance program. Adhere to all ECHS policies and procedures including the Code of Conduct, Confidentiality, and HIPPA.
- Acts in an ethical, legal, and responsible manner, following all applicable ECHS policies, state/federal laws, regulations, and reporting requirements.
- Respects diversity of others by demonstrating and modeling cultural awareness and sensitivity at all times.
